O diagnóstico é feito essencialmente através da conversa e observação pelo médico, com o auxílio de alguns questionários clínicos. É difícil de estabelecer e muitos doentes são vistos por vários médicos até saberem que têm fibromialgia.
Devem ser excluídos outros possíveis diagnósticos através de análises ao sangue e outros exames.
